The fit difference
A custom mouthguard starts with a digital or putty impression of your specific teeth. It's then fabricated layer by layer in a dental lab to fit every contour of your dental arch. The result is a guard you barely notice, it stays in place, doesn't shift when you talk or breathe, and contacts each tooth evenly.
A drugstore boil-and-bite gets close but never quite there. They're bulky, often loose, and pressed unevenly into a generic mold. Most athletes who try both never go back to OTC.
Effectiveness in real impact
Sports dentistry research has compared injury outcomes between custom and OTC mouthguards. Custom guards reduce the risk of dental injury 60× over no guard at all. OTC guards reduce it 4–5×, better than nothing, but a fraction of what custom protection provides. The reason: custom guards stay seated during impact instead of dislodging.
