This Privacy Policy describes how Cusp Dental ("we," "our," "us") collects, uses, and protects information you provide through this website. We respect your privacy and are committed to handling your data responsibly.

Information we collect

We collect personal information you voluntarily provide, such as your name, phone number, email address, and any messages, when you submit a contact or appointment request form, call us, or correspond with us by email. We also collect non-identifying browsing information (device type, browser, pages visited, approximate region) to understand how the site is used and to improve it.

We do not knowingly collect sensitive personal information through this website. Protected health information ("PHI") collected during your care is handled separately under our HIPAA Notice of Privacy Practices.

How we use information

We use the information you provide to:

  • Respond to your inquiries and confirm or schedule appointments.
  • Verify insurance benefits when you request it.
  • Send transactional communications related to your visit (appointment confirmations, reminders, follow-ups).
  • Improve the content and performance of this website.
  • Comply with legal obligations and protect against fraud or misuse.

We do not sell or rent personal information to third parties, and we do not use the information you provide here for cross-context behavioral advertising.

HIPAA and protected health information

Protected health information collected as part of your dental care is governed separately by our Notice of Privacy Practices, which is provided at your first visit and is also available on request. This Privacy Policy applies to information collected through this website only.

How we protect your information

Information submitted through the website is transmitted over an encrypted connection (HTTPS / TLS). We limit access to information you submit to staff who need it to respond to your request, and we retain it only for as long as needed to fulfill the purpose for which it was provided or as otherwise required by law. We cannot guarantee absolute security, but we use commercially reasonable safeguards consistent with the sensitivity of the information.

Children's privacy

This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the website. Parents and guardians scheduling pediatric appointments are encouraged to do so on the child's behalf. If you believe we have inadvertently received information from a child under 13, please email us and we will delete it.

Cookies and analytics

This website uses Google Analytics 4 (measurement ID G-LCLHSPPG25) to understand how visitors use the site so we can improve it. Google Analytics sets first-party cookies (_ga, _ga_*) that store an anonymized visitor identifier; IP addresses are anonymized before processing. We do not enable Google Signals, advertising features, or remarketing on this property. We do not run Google Ads pixels or third-party advertising tags.

You have several ways to opt out:

  • Click Decline on the cookie banner that appears on your first visit. We will stop sending data to Google Analytics for your browser and delete any analytics cookies that were already set.
  • Enable the Global Privacy Control (GPC) signal in your browser (built in to Firefox, Brave, DuckDuckGo Browser, and others, or via the Privacy Badger extension). We treat a valid GPC signal as an automatic opt-out and the cookie banner will not be shown.
  • Re-open the cookie banner anytime using the "Cookie preferences" link in the site footer to change your choice.
  • Submit a formal request through our Do Not Sell or Share My Personal Information page.

The accessibility widget stores your display preferences (text size, contrast, motion, etc.) in your browser's localStorage. These are essential preferences, not tracking, and remain regardless of your analytics choice.

Third-party services

This website embeds a Google Maps preview of the practice location and may embed a YouTube background video on certain pages. The Do Not Sell page embeds a third-party form provided by JotForm to receive opt-out requests. Each of these services may receive your IP address and may set their own cookies when their content loads. Each service has its own privacy policy governing the information it collects.

Information we collect, categories at a glance

  • Identifiers (name, email, phone number), collected directly from you when you submit a form or contact us. Used to respond to your inquiry and schedule care.
  • Internet or other electronic network activity (browser type, device type, pages visited, approximate region, anonymized analytics identifiers), collected automatically when you visit. Used to improve the website's performance and content.

We retain identifiers only as long as needed to respond to your request or as required by law (typically up to two years after your last interaction). We retain analytics data on the schedule configured in our Google Analytics account.

Your rights

Depending on where you live, you may have the right to know what personal information we hold about you, to request a copy of it, to ask us to correct or delete it, to limit how we use it, and to opt out of any "sale" or "sharing" of it. California residents can also exercise these rights through the Do Not Sell or Share My Personal Information page. To make a request directly, email us. We will respond within a reasonable time consistent with applicable law (in California, generally within 45 days, with one allowed extension).
